Tuméfaction nasogénienne: ne pas oublier un kyste nasolabial

Nasolabial cyst or cyst invading the nasal fold is rare but it should be suspected in patients with nasal vestibule cysts. It is a non-odontogenic cystic lesion of epithelial origin characterized by extra bone growth on the maxilla. The diagnosis of nasolabial cyst is strongly suspected clinically and the size of the cyst can reach 3 to 4 cm resulting in facial deformation. CT scan confirms the diagnosis and evaluates nasolabial cyst relationship with adjacent structures. Patients are treated surgically and report a low recidivism rate. Vestibular surgery is the treatment of choice and allows total cyst resection. We report the case of a 64 year-old female patient presenting with left swelling involving the nasolabial region evolving for 16 years. Swelling had gradually increased in size and was associated with progressive worsening of left nasal obstruction. Clinical examination showed renitent swelling invading the left nasal fold, extending to the infraorbital fold and at the level of the left nasal floor measuring 5 cm along its longer axis. Maxillofacial CT scan showed cyst formation in the nasal vestibule with large footprint on the neighbouring maxillary bone. The patient underwent vestibular surgery for the excision of a large cyst adhering to the left nasal floor and whose dissection in its upper part proceeded from neighbouring infraorbital foramen. The postoperative course was uneventful. Anatomo-pathological examination confirmed the diagnosis of nasolabial cyst. No recurrence was noted after a follow-up of 3 years.
